A New Look for Sports Forms!

We are pleased to introduce our new sports forms - a condensed version with a brand new look.

It is necessary to fill out only ONE of the signature pages per school year. The signature page is good for all athletics, band and extracurricular clubs for the entire school year. Please note that there are FIVE signature lines on this form.

Students may need one or more yellow Medical Emergency Information / Medical Authorization cards depending on how many sports or activities they participate in.

Most of our forms can now be downloaded below. The yellow cards are available in the Main and ASB Offices, as are complete sports packets, during the school year.
